    Title: Genotypic and phenotypic characterization of extended spectrum beta-lactamases producing Klebsiella pneumoniae and Escherichia coli strains in clinical specimens

    Abstract: Background: The prevalence of extended-spectrum beta-lactamase-producing organisms and their antimicrobial resistance patterns may vary between genotypes. Objectives: To evaluate the genotypes and susceptibility of 14 K. pneumoniae and 39 E. coli isolated from patients in hospital. Methods: Antibiotic susceptibilities were determined by disk diffusion confirmatory test and VITEK II compact system according to CLSI standards. PCR and DNA sequencing were performed using bla SHV-1, bla SHV-5, bla SHV-12, bla CTX-M3, bla CTX-M9, bla CTX-M14, bla TEM-109, bla DHA and specific primers, with DNA obtained from the clinical strains. Results: In this study showed that the 31 ESBL-producing K. pneumoniae and E. coli strains, the bacterias had drug resistance, including with Ampicillin, Piperacillin, Cefazoline, Ceftazidime, Ceftriaxone, and Cefepime. On the other hand, carried the drug resistance genes was compared based on PCR and DNA sequencing methods between strains from the clinical strains, 7 strains (22.58%) had the blaSHV-1 genotype, 8 strains (25.81%) had the blaSHV-5 genotype, 6 strains (19.35%) had the blaSHV-12 genotype, 9 strains (29.03%) had the blaCTX-M3 genotype, 24 strains (77.42%) had the blaCTX-M9 genotype, 11 strains (35.48%) had the blaCTX-M14 genotype, 25 strains (80.65%) had the blaTEM-109 genotype, and 1 strain (3.23%) had the blaDHA genotype. Conclusion: The prevalence of ESBL in K. pneumoniae and E. coli, its resistance pattern is noticeable, as well as genetic elements involved in the assembly and spread of multidrug resistance.
